Tissue-resident immune cells in human chronic liver diseases

Summary

Although not commonly thought of as an immunological organ, the liver contains large numbers of immune cells and many pathological processes in the organ are direct consequences of misdirected immune responses. In particular, the human liver is selectively enriched for natural killer (NK) cells, innate lymphocytes important during infections and in inflammation, representing up to 40% of all lymphocytes at the organ. The main objective of this project is to investigate NK cell development and functional regulation in the healthy human liver and to study their role during settings of liver disease. My position as a physician at the Hepatology clinic at Karolinska (20% basis) allows unique access to human liver and bile duct material. Furthermore, we have developed unique state-of-the-art tools (23-parameter flow cytometry, bioinformatics analysis tools), suitable for studies of rare and limited patient material that will be utilized in the project. Specific aims include identifying and characterizing NK-cell progenitors in the healthy human liver. Further, we aim to determine the role of bile duct-resident lymphocytes in biliary disorders. Finally, we will study phenotypical, functional, and transcriptional aberrancies of NK cells in an emerging chronic liver disease, steatohepatitis, and during intrahepatic tumor development. We anticipate generating new understanding of basic NK-cell biology in the liver as well as on the role of intrahepatic NK cells during liver disorders.

Den övergripande målsättningen med aktuellt projekt är att studera så kallade naturliga mördar celler (NK celler), en del av vårt immunförsvar, och dessa cellers roll i den mänskliga levern. NK celler är viktiga för att bekämpa virusinfektioner samt tumörsjukdom. Dock kan de också bidra till inflammation. Tidigare har man fokuserat mycket på att studera NK celler i blod samt även i olika musmodeller. Ett intressant fynd är att NK celler är en av de mest förekommande immunologiska cellerna i den mänskliga levern. Detta vill vi studera i aktuellt projekt. Vi kommer använda unikt material från levern hos patienter med specifika leversjukdomar och med hjälp av så kallad avancerad flödescytometri i detalj kartlägga NK cellerna funktion samt hur denna funktion regleras. Vår målsättning är att nå ny kunskap kring hur NK celler fungerar i den friska levern. Vidare är målsättningen att förstå vilken roll NK celler har i dels så kallad fettleverhepatit och dels i samband med levercancer. Vi förutspår med aktuell studie att vi kommer identifiera nya immunologiska faktorer och signaleringsvägar som framöver kan bli måltavlor för nya behandlingar eller som kan användas av sjukvården för att bättre prognosticera leversjukdomar.
